prepares you for the first 48 hours of breastfeeding.

Not because the information is not available. There is plenty of information. But most of it is delivered before the birth, when you are still pregnant, when the baby is still abstract, when your capacity to absorb and retain what you are being told about cluster feeding and latch and colostrum is limited by the fact that none of it is real yet.

And then the baby arrives. And everything you read becomes something you have to implement in a body that has just done the most physically demanding thing it has ever done, in a state of exhaustion unlike anything you have experienced before, with a tiny person who is hungry and whose hunger is now entirely your responsibility.

This article is for the version of you that is in those first 48 hours right now, or the version that is about to be. It is also for the version that is still pregnant and reading everything she can find, because reading this now, when you are not yet in the middle of it, is genuinely useful preparation even if it feels distant.

Here is what those first hours and days actually involve.

Skin-to-skin Contact: The first moments after birth

If your birth goes as planned and you and your baby are both well, the first thing that should happen after delivery is not the weighing, not the measurements, not the footprints. It is your baby placed on your chest, skin to skin, before any of those things.

Skin-to-skin contact in the first minutes and hours after birth is not a preference or a nice-to-have. It is the most physiologically significant thing that can happen for your baby in those first moments, and it is also one of the most significant things for the establishment of breastfeeding. The Golden Hour as they say.

What it does for your baby: baby’s body temperature stabilises against yours rather than dropping in the air outside the womb. Their heart rate and breathing regulate. Their blood glucose, which newborns need to maintain immediately after birth, is supported by the closeness and warmth and the colostrum that they will instinctively move toward. Baby’s cortisol, the stress hormone elevated by the significant physical experience of birth, falls. Baby’s microbiome begins its first colonisation from your skin. All of this happens in the first minutes of skin-to-skin contact, before a single intervention.

What it does for breastfeeding: the rooting reflex, the instinctive searching and nuzzling movement that leads a newborn toward the breast, is strongest in the first hour after birth. This first hour is sometimes called the golden hour precisely because the hormonal and reflexive conditions for the first breastfeed are most favourable in this window. A baby placed skin-to-skin on the mother’s chest will often move toward the breast and attempt to latch without being directed, following the same instinct that has worked for every mammal before her.

If you have a caesarean birth, skin-to-skin is still possible and still important, though the logistics are different. In many Mauritius operating theatres it can happen on the table if you and your baby are both well. If this is your plan, discuss it with your gynaecologist before the birth and have it noted in your birth record so the team knows your wishes before the moment arrives.

The time pressure on this first hour matters because interventions that are not medically urgent, the bath, the measurements, the administrative procedures, can wait. The first breastfeed cannot wait as effectively. If someone in the delivery room suggests moving your baby for something non-urgent in the first hour after birth, you are entitled to ask whether it can wait until after the first feed.

The first breastfeed

The first breastfeed often does not look like what you imagined it would look like. Oh Giiiiirl…

Your baby may latch immediately and feed confidently. This happens and it is wonderful when it does. It is also not the universal experience and its absence does not signal a problem.

More commonly, the first breastfeed is a negotiation. Your baby is new to this. You are new to this. The latch may take several attempts. Your baby may fall asleep before completing a feed, because the effort of birth and the warmth of your chest and the overwhelming sensory experience of being outside the womb for the first time combine to produce a drowsiness that is normal and temporary. Baby may root enthusiastically and then lose the latch and become frustrated. You may find the positioning difficult, your arms different from how you imagined them, your breast different from what the diagrams suggested.

None of this is failure. The first breastfeed is the beginning of a learning process for both of you, and learning processes have a developmental arc that takes days to weeks to resolve into something that feels natural rather than effortful. The breastfeeding that feels effortless at six weeks looks nothing like the first breastfeed at six hours, and the distance between them is almost entirely a matter of practice, patience, and adequate support.

What matters most in the first breastfeed is not perfect latch technique but the attempt. Your baby needs colostrum, and she needs the stimulation to the breast that tells your body to continue producing it. Both are served by bringing her to the breast, doing the best you can with the information and support available to you in the room, and continuing to try even when it does not go smoothly.

Ask for help in the room. Every midwife and nurse who attends births should be able to offer at least basic breastfeeding positioning support. If the person in your room is not able to help effectively, ask for someone who can. You are not asking for too much. You are asking for something that directly affects your baby’s nutrition.

Colostrum

Before your mature breast milk arrives, your body produces colostrum. It has been present in your breasts since around week 16 of pregnancy, which is why some women notice a yellowish fluid from their nipples in the third trimester. After birth, it is what your baby receives in every feed for the first two to four days.

Colostrum is yellow or orange in colour, thick in consistency, and produced in very small volumes. Very small. In the first 24 hours, a breastfed baby may receive only a few millilitres per feed, which sounds alarming until you understand why it is exactly right.

A newborn’s stomach at birth is approximately the size of a marble. It holds between 5 and 7 millilitres. The small volume of colostrum your body produces in the first days is precisely calibrated to that stomach capacity, not insufficient, not a sign that your milk is not coming, but the appropriate amount for the appropriate organ at the appropriate developmental stage. This is one of the most consistently misunderstood aspects of newborn feeding, and it is the source of enormous anxiety and unnecessary supplementation in the first days of life.

Colostrum is not a placeholder for real milk. It is itself a complete and specific nutrition and immune source designed for the newborn period. Its concentration of immunoglobulins, specifically secretory IgA, provides a layer of gut lining protection that is sometimes described as the baby’s first vaccine. Its concentration of protein, zinc, and other immune-active compounds is higher per millilitre than mature breast milk will be, because it does not need to provide volume. It needs to provide protection and the specific nutrients most critical in the first days of life.

The reason frequent feeding matters in these first days, and the reason that keeping your baby at the breast often even when the feeds feel short and unproductive, is that the stimulation of frequent feeding is the signal that drives the transition from colostrum to transitional milk and then to mature milk. The more frequently and effectively your baby feeds, the sooner and more abundantly your milk will come in. The feeds that feel pointless because they are brief and you cannot see or measure what your baby is getting are not pointless. They are doing the most important work of these first days.

When the milk comes in

Most women experience the transition from colostrum to mature milk between day two and day five after birth, though for some women it takes slightly longer, particularly after a caesarean birth or a difficult delivery.

The milk coming in is not subtle. Most women know when it has happened because their breasts become noticeably fuller, heavier, and firmer, sometimes significantly so. Some women describe the feeling as engorged, which is the word used when the breasts become very full, hard, and uncomfortable as the initial rush of milk production adjusts to the actual demand. Engorgement typically resolves within a day or two as supply and demand begin to calibrate. Feeding frequently, including at night, is the most effective response to engorgement because emptying the breast is what signals to your body the appropriate volume to produce.

In Mauritius, where the heat can be significant, the fullness of engorgement can feel more intense than it might in a cooler climate. Cold compresses applied to the breasts between feeds reduce discomfort. Warm compresses immediately before a feed can help the milk flow more easily. Both are available with a cloth and water at home and do not require any specific product.

The milk that arrives in the first days is transitional milk, which looks different from what you may expect. It is thin and can appear almost watery or bluish white rather than the creamy consistency of colostrum. This is normal. The composition of breast milk changes dynamically throughout a feed and throughout the weeks of breastfeeding. What your baby receives is exactly appropriate to her needs at each stage regardless of what it looks like.

What the first 48 hours actually feel like

This section is the one most pregnancy content leaves out, because it is the hardest to describe in a way that does not sound alarming, and also because it is the most useful information for a woman about to experience it.

The first 48 hours after birth are, for most women, the most physically and emotionally overwhelming period of their life. You have just given birth, which is a significant physical event by any measure. You may have had very little sleep in the hours surrounding it. You are now responsible for feeding a newborn whose hunger is immediate and frequent and whose signals you are still learning to read. Your hormones are shifting dramatically, which produces emotional responses that feel disproportionate to their triggers and that can include crying that arrives without a clear reason, a feeling of being completely overwhelmed, a strange combination of love and terror and exhaustion and something that does not yet have a name.

All of this is normal. It does not mean you are struggling. It does not mean you are not coping. It means you are a person who has just done something enormous and is in the immediate aftermath of it.

The breastfeeding in the first 48 hours is frequent, because newborns feed every one to three hours and because their stomach capacity means feeds are short and need to be repeated often. The feeds that happen at 2am and 4am and 6am are not a sign that something is wrong with your supply or your baby. They are developmentally appropriate newborn behaviour that will change as your baby grows and their stomach capacity increases. In the first 48 hours, feeding often and resting whenever you are not feeding is the entire instruction.

The people around you in the first 48 hours, the nursing staff at your clinic, your partner, your family, will have opinions about whether your baby is getting enough. These opinions, however kindly intended, are not always accurate and not always helpful. The evidence-based markers for whether a newborn is adequately fed in the first days are not how settled they seem or how long they sleep or whether they stop crying when someone other than you holds them. They are wet nappies, dirty nappies, and weight trajectory assessed by a healthcare professional. Everything else is noise.

The people around you and the comments about formula

In Mauritius, the early postpartum period often involves significant family presence, which is a genuine source of support and warmth and also occasionally a source of pressure around feeding. The mother-in-law or grandmother who suggests that the baby is not getting enough, that formula would settle your baby, that babies need more than what breastfeeding provides in the first days, is usually speaking from genuine concern. She is also almost certainly reflecting the information that was available when she was raising her own children, before the research on colostrum volumes and newborn stomach capacity was widely disseminated.

The most effective response to feeding-related pressure in the first 48 hours is not argument, which requires energy you do not have, but redirection to a professional. If there is a concern about whether the baby is getting enough, the appropriate person to assess it is the midwife or nurse in the clinic or a lactation consultant, not the family member in the waiting room.

If formula is recommended to you in the first days without a clear clinical reason by someone who is not a healthcare professional, you are entitled to ask for a healthcare professional’s assessment before acting on it. Supplementation in the first days, when colostrum volume is appropriately small and the supply-establishing mechanism of frequent feeding is most active, can disrupt that mechanism in ways that affect breastfeeding success beyond the first days. This is not to say supplementation is never appropriate. It is sometimes clinically necessary and the right decision. But it is a clinical decision that deserves clinical assessment rather than social pressure.

Breastfeeding in the first days should not be significantly painful. Mild discomfort and sensitivity as the nipples adjust to a demand they have not previously experienced is common and normal. A sharp, persistent pain during feeding, a pain that makes you dread the next feed, cracked or bleeding nipples, and shooting pain between feeds are not normal and are almost always signs of a latch issue that is addressable rather than something to push through.

The most important thing to know about breastfeeding pain is that it is not a sign that you are not built for breastfeeding or that your body is not capable of it. It is almost always a sign that something about the positioning or the latch can be adjusted, and that adjustment typically makes an immediate and significant difference.

Ask for help before the pain becomes something you are managing rather than something you are solving. A lactation consultant, either through the clinic or independently, can assess the latch in real time and make specific, practical adjustments that no amount of reading can replicate.

Looking beyond 48 hours

The first 48 hours are the most intense part of the learning curve, but they are not representative of what breastfeeding will become. The breastfeeding that is established and working at six weeks looks almost nothing like the breastfeeding of the first two days. The latch that takes multiple attempts per feed in the first days becomes something that takes seconds. The feeds that feel endless become shorter and more efficient as your baby grows and her stomach capacity increases.

What makes the difference between a breastfeeding experience that works and one that does not is almost never physiology. Most women are physically capable of producing adequate milk for their baby.
